Hair loss treatment: scalp solution or capsule?
A daily scalp solution and a once-a-day capsule take different routes to pattern hair loss. Here’s what’s in each, what to expect and who shouldn’t use them.
In short
- Thinning at the temples, crown or part is often pattern hair loss, but other kinds look similar, so a provider checks your scalp photos first.
- Hair Regrowth Solution is a daily scalp solution for adults, and Minoxidil + Dutasteride is a once-a-day capsule for men only.
- Plan on about 6 months before judging either one, expect a possible early shed, and know that gains usually fade within a few months of stopping.
- Both are compounded and use ingredients off-label, neither combination has been tested in its own trial, and a provider decides what’s right for you.
First, what kind of hair loss is it?
Pattern hair loss runs in families. In follicles that are sensitive to it, a hormone called DHT shrinks the follicle a little with each growth cycle, so hairs grow back thinner and shorter. It affects up to half of men by 50, and a third or more of women by their 60s.
Other kinds of hair loss can look alike: alopecia areata, scarring alopecias, scalp ringworm, hair loss from tight styles, and thyroid or iron problems. Minoxidil isn’t the answer for them, so a provider checks scalp photos first. Temporary shedding after an illness, surgery or fast weight loss usually settles within about 6 months, and shedding after a birth by the baby’s first birthday.

Hair Regrowth Solution pairs minoxidil, which helps follicles spend longer in their growth phase, with tretinoin. In small studies, tretinoin raised the scalp enzyme that activates minoxidil, and it lets more minoxidil through the skin. Minoxidil is FDA-approved for hair loss on its own, as in Rogaine; this combination isn’t. Clascoterone is FDA-approved as a 1% acne cream (Winlevi), not for hair loss. Your provider chooses the formula with you.
Minoxidil + Dutasteride is one capsule. Dutasteride blocks both forms of the enzyme that makes DHT, so it lowers DHT further than finasteride. By mouth, minoxidil is FDA-approved only as tablets for severe high blood pressure, usually at 10 to 40 mg a day. Dutasteride is FDA-approved in the US for an enlarged prostate, not hair loss.
For men, the FDA-approved hair-loss medicines are minoxidil on the scalp and finasteride 1 mg, and your provider may recommend one instead.
What the research shows
Scalp solution
In trials in men and women, minoxidil grew more hair than a placebo, and in a 48-week trial of 381 women, 5% grew more than 2%. Tretinoin with minoxidil has only small trials: in one of 31 men, once-daily minoxidil with a weaker tretinoin did about as well as minoxidil twice a day. In two large trials in 1,465 men, a 5% clascoterone scalp solution grew more hair than a placebo solution at 6 months. Those results come from the drug’s maker, aren’t fully published, and include no women.
Capsule
In a 24-week trial of 917 men aged 20 to 50, dutasteride 0.5 mg a day grew more hair than a placebo and more than finasteride 1 mg. In a 24-week trial of 90 men, oral minoxidil 5 mg a day did about as well as minoxidil solution twice a day. Benefit rises with dose, and lower doses like the capsule’s 2 mg have been tested only in small trials.
Where the evidence stops
Neither combination has been tested in its own trial, including oral minoxidil with dutasteride. The dutasteride trials used the FDA-approved soft capsule, and most trials lasted about 6 months.
How long to wait, and the early shed
- Weeks 2 to 8: some people shed more at first, as resting hairs make way for new ones. It usually settles within a couple of months, and stopping during the shed resets the clock.
- Months 3 to 4: some people see fine, short new hairs. Results vary.
- Month 6: the first fair point to judge the scalp solution. The capsule takes 6 to 12 months of daily use to judge.
Monthly photos in the same light help you compare. Areas bald for years rarely regrow, and since pattern hair loss keeps going without treatment, holding steady counts. Hair regained usually returns to its earlier pattern within a few months of stopping.
Side effects
- Scalp solution: mild dryness, itching or flaking is the most common. Fine facial hair can grow if the liquid runs onto your face; in the 381-woman trial, it happened in 4, all on 5%. Tretinoin makes skin burn more easily in the sun. Stop and tell your care team if your scalp gets red, raw or painful, or if you feel dizzy.
- Capsule, minoxidil: extra hair on the face and body is the most common. Less often: light-headedness, swelling of the ankles or around the eyes, a faster heartbeat or headache.
- Capsule, dutasteride: a small share of men get a lower sex drive, erection or ejaculation problems, or tender or larger breasts. In trials in older men with enlarged prostates, erection problems affected about 5% in the first 6 months, against 2% on placebo. For some men, sexual side effects have continued after stopping. Low mood has been reported.
Safety and who shouldn’t use them
A provider reviews your health history, medicines and scalp photos first, and prescribes only if it’s right for you. Tell them about heart disease, low blood pressure or blood pressure medicine.
The scalp solution
- Not during pregnancy, while trying to conceive or while breastfeeding. Clascoterone blocks male hormones, so keep your treated scalp, hands and pillowcase away from a pregnant partner’s skin.
- Minoxidil is very toxic to cats and dangerous to dogs, and babies have grown extra body hair after touching a parent’s treated scalp. Let it dry, wash your hands, and keep pets and babies off your pillow.
- Don’t apply it to a red, inflamed, infected, irritated or painful scalp.
- Stop and get checked for chest pain, a racing heart, faintness, swelling or sudden weight gain.
The capsule
- It’s for men only. Anyone who is or may become pregnant shouldn’t take it or touch a broken or leaking capsule, because dutasteride passes through the skin and can harm a male baby. After contact, wash the skin with soap and water right away.
- Don’t donate blood while taking it or for 6 months after your last dose.
- Don’t take it with finasteride, after an allergic reaction to either medicine, or if you have pheochromocytoma, a rare adrenal tumor.
- Swallow capsules whole. Don’t open, crush or chew them.
- Dutasteride roughly halves PSA results; tell anyone ordering a prostate test.
- Tell your provider about liver disease, or drugs such as ketoconazole or ritonavir.
- Get care right away for chest pain, trouble breathing (especially lying down), fainting, a resting pulse 20 or more beats above your normal, a quick weight gain of 5 pounds or more, or new swelling. Call or text 988 if you have thoughts of harming yourself.
